Surveys

We encourage all residents to fill out this survey as soon as possible. The data collected will be kept confidential and will only be used during an emergency by City staff and your district representatives.

The survey will be used for the following:

  • Locate members of the community that may be missing
  • Identify those who require special medical attention
  • Help evacuate those who may be injured
  • Coordinate the evacuation of Horses and household pets
  • Identify utility (gas, water, electricity) shut off valves
  • Match those members of the community that may be able to offer ser- vices/skills/equipment with those in need

As seen with Hurricane Katrina, assistance from the "First Responders"; fire, paramedic and law enforcement may not arrive for several days in a major catastrophe. You will need to depend on your neighbors and your local emergency response committee members until they arrive. This survey maybe our only link to reach you.
